There's also a small chance that the recipient's e-mail account has been hacked and that whoever did it turned on auto-forwarding, routing all incoming mail to some other host.

...I believe that is exactly how 'Windows Live Mail' is designed to work. So's the rest of the Windows Live programs.

User-defined filters with insufficient specificity can route incoming e-mails to the bit bucket before they're ever read. E-mail hosts can flag incoming mail as spam even though it's not. Yahoo is particularly bad about that.
