When was Medicare supposed to pay it all? Was there such a thing?

...if you have to have a $100,000 operation, medicare may pay 80% of it. Whatever they don't pay you are responsible for. If you don't have a supplemental that pays $20,000 it's yours to pay. You 'silver sneakers' membership is useless.
