Hi CF, I love your idea, it could even be an "opt in" program for those that like it so much. Medicare is very fixable w/o such extremism. How about allowing the gov't to purchase drugs on bid for the most obvious one. Medicaid is different and implementing a voucher system for those recipients may be worth a look see. In my county, we opened an Urgent Care Clinic in early 2010. It saved taxpayers over $1M in it's 1st year. Medicaid recipients were encouraged to use it and did. Hospitals saved money, emergency room use is down and more efficient. Win, win, win. A voucher system for medicaid might encourage wider adoption and use of such clinics.
