...I think it is the History Channel that has the running series on aircraft disasters. On a crash quite similar to this one (engine catching fire and falling off the airplane) they found that there was a very expensive machine and it was very time consuming to take the engines off an airplane for maintenance so some of the maintenance crews were using a forklift to put the engine on the bolts that hold it to the pylon and then torquing it into place. The accident I have described was repeditive. Could be the same thing here in this case. Pics of cracks eerily similar.
