My wife's nephew is a pastor of a small evangelical church in Washington state and they are the same way. No masks and while they've not done much in the social distancing side, weather permitting, they at least hold Sunday services outside.
But they're absolutely against the vaccines, claiming that Bill Gates is going use it to track people due to the embedded 'microchips', and besides, the vaccine will leave the 'mark of the beast' on anyone who gets the shot. And he preaches this from the pulpit (my wife watches his services as he live-streams them).
He's also is convinced the Trump won the election and the God will soon restore him to office.
I'm not sure how many people in his church has had the COVID, but it has been several.
His wife is the same person who told my wife over a year ago, before the election, that they had to vote for Trump because if the Democrats win, they'll take away their health insurance (they have a daughter who was born with a heart defect). That it was Trump and the Republicans who provided them with preexisting condition protection that Obama and the Democrats had tried to take away from them. And as you'd expect, they only watch Faux News and now OANN.
